504 Parent Rights & Procedural Safeguards

 

Referral: Parents/caregivers/guardians or school school staff members who suspects a student may have a disability requiring accommodations or related services may make a referral to consider 504 eligibility. The student’s school counselor oversees this process for 

 

Evaluation & periodic review: Multiple sources of data are considered to determine eligibility for a 504 plan and to make revisions to the 504 plan. This can include specific assessments, and other data included but not limited to teacher reports, classroom observations, grades, attendance records, health information, classroom participation and performance

 

Eligibility determination: Students are determined eligible for accommodations through a 504 plan, following a team meeting that considers referral concerns and a range of data sources. 

 

If a student enrolls at Lincoln Sudbury with a 504 plan in place, the district will honor the 504 as written, and then review the 504 plan after a period of time to assess whether a student still requires accommodations based on their identified mental or physical impairment. If a parent/caregiver has any questions, they should reach out to their student’s school counselor.
